Advanced Practice Provider (APP) Primary Care with Hormone Therapy Experience

Full-time and part-time positions available.

What You Can Expect in This Role

  • Provide comprehensive, integrative care across multiple settings — in-office and via telehealth
  • Support women through perimenopause, menopause, and related hormonal transitions by integrating HRT management into preventive and lifestyle-based care.
  • Evaluate, diagnose, and treat patients following evidence-based care pathways, including management of chronic conditions, preventive care, and hormone optimization.
  • Collaborate with physicians and specialists on complex cases to ensure holistic treatment plans.
  • Guide patients through their wellness journey, educating them on lifestyle, nutrition, hormone balance, and preventive health strategies.
  • Communicate proactively and compassionately with patients to deliver an exceptional experience.
  • Conduct health risk assessments, manage ongoing treatment plans, and ensure continuity of care.
  • Leverage technology and integrated EMR systems to streamline workflows and maintain documentation accuracy.

Schedule · Monday–Friday· No weekends or evenings.

Requirements

· 3+ years of clinical experience, preferably in primary care, endocrinology, GYN, or restorative health

· Demonstrated experience in hormone therapy (bioidentical hormone replacement, anti-aging medicine, functional medicine)

· Master’s degree from an accredited NP or PA program

· Active, unrestricted Georgia NP or PA license

· Active or eligible DEA license in Georgia

· Ability to work in the Atlanta, GA (Old Fourth Ward) office
