DESCRIPTION:

The Pharmacist 5 in Transplant is a recognized organization-wide expert and has significant impact and influence on organizational policy and program development. They are the experts on drug therapy and the primary health professionals who optimize use of medication for the benefit of the patients. Roles of the pharmacist in this setting include: meets educational needs of patients, healthcare team, students and residents; ensures adherence to regulatory and fiscal requirements; regularly leads projects of critical importance to the organization; directs programs with organization-wide impact that include formulating strategies and administering policies, processes, and resources. Partners with physicians and other clinicians to provide treatments and advice to patients and records patient history relating to medication use. Advises physicians in determining the best possible treatments for specific diseases, stays current on pharmacy regulations, treatments and developments in research and design. The Pharmacist 5 in Transplant is the recognized professional authority in drug therapy for management of disease states and optimization of pharmacotherapy in abdominal and cardiothoracic transplant and mechanical assist devices. They require the application of advanced intensive knowledge of immunosuppressive therapy gained through education and specific training transplant, as well as significant experience in management of patients in ambulatory care settings. This position will cover pharmacist roles in heart, lung, kidney and liver transplant program settings within the inpatient, transitions of care, and ambulatory environments.

This position has recently been accreted by the HX union, and will be a part of that union moving forward.

Depending on experience of the selected candidate, this position will be hired at the Stf Pharm2 or Stf Pharm5. Less than seven (7) years experience qualifies as Stf Pharm2; Seven (7) + years of experience qualifies as Stf Pharm5.

MINIMUM QUALIFICATIONS:

  • Doctor of Pharmacy degree (Pharm D) or equivalent training.

  • One of the following: Successful completion of a PGY2 specialty residency in the area of specialization + one (1) year of relevant practice; Successful completion of a PGY1 pharmacy practice residency + four (4) years of relevant practice; or seven (7) years of dedicated practice in transplant if there is no advanced pharmacy training.

  • Registered Pharmacist - issued by the state of California (RPH) in good standing.

  • Board Certification in Pharmacotherapy and/or Board Certification in Specialty Practice. (Pharm5)

  • Knowledge of developments in research and design in area of specialization including new or reformulated products being developed by medical and pharmaceutical researchers.

  • Strong communication skills and the ability to build positive, trusting relationships with providers, health care staff, and learners.

  • Strong knowledge and experience in medication use processes, including medication access strategies.

  • Highly detail oriented, motivated, and independent.

PREFERRED QUALIFICATIONS

  • Proficiency in using EPIC electronic medical record.
  • Clinical pharmacy experience in the inpatient, transitions of care, and outpatient settings.
  • Excellent attention to detail, organizational skills, and the ability to prioritize tasks effectively.
  • Ability to work independently and collaboratively within a team-oriented environment.
  • Familiarity with prior authorization processes and insurance coverage verification.
  • Documented record of improvement in and contributions to the respective area of specialty practice through CQI projects, participation in nursing in-services, protocol development, etc.
  • A sustained record of contributing to the total body of knowledge in the specialty area of pharmacy practice annually through (1) research, (2) publications in professional journals, and/or (3) presentations at professional meetings.
  • Proven leadership in advancing the profession of pharmacy through active service in professional organizations at the local, state, and national levels.
  • Demonstrated commitment to teaching (e.g. through preceptorship, education of medical center staff through clinical forums, grand rounds, etc.).
  • Medication-use evaluation experience.
