Silver Spring Healthcare Services in Spring, TX, is seeking a compassionate and skilled LVN/RN to make a real difference in people's lives as a dedicated Home Health LVN/RN on a PRN basis. With the opportunity to provide top-notch care to patients in the comfort of their homes, you can earn $45.00 (LVN) - $50.00 (RN) per visit for 3 visits per week. Educate patients and their families on disease management and ways to promote overall well-being.
Work collaboratively with diverse healthcare teams to ensure effective coordination of patient care.

Qualifications:

Must have at least 2 years of experience working in home health care as an LVN/RN in Spring, TX, US.
Required to possess an active LVN or RN license, specifically in the state of Texas.
Proficient in the performance of patient assessments and the development of personalized care plans.
Strong communication skills are essential, both verbal and written.
Capable of working autonomously and collaboratively within a multidisciplinary healthcare team.
