Visiting Clinicians, P.C. is a provider of general medicine and related clinical services, specializing in home-based visits. As an employee of Visiting Clinicians, you will be a part of providing high-quality, safe, and affordable care to patients in the comfort of their home.

Every Saturday and Sunday, 7A-7P

Up to 29 hours weekly

Attractive pay

• $38.00 to $40.00/Hour

• Connect with providers and patients when change in condition requires escalation to a treating provider.
• Follows escalation process specific and coordinate with appropriate on-call clinician, provide report on patient condition, and carry out any orders or changes to treatment plan.
• Maintains all required documentation on provider and patient interactions including visits, intake, interventions, patient issues and any other elements of the patient's individual plan of care.
• Manages clinical and operational workflows related to patient change in condition or clinical needs.
• Builds and maintains collaborative professional working relationships with physicians in each of the markets.
• Performs other duties as assigned.

• Associate of Science in Nursing.
• Current, active RN licensure in good standing and willing to be licensed in multiple states at Contessa's expense. Compact License is required, New York License preferred. Michigan license is a plus.
• Three to five years of acute nursing experience.
• Triage experience is preferred.
• Possess a general knowledge of clinical standards and outcome measurement.
• Must have reliable transportation and proof of liability insurance.
